SAFETY CRITICAL INDUSTRIES

Errors within the aviation industry and their consequences are very visible and have always been the focus of public and media attention, often described as 'Pilot Error'. In the past the response has usually been a rewritten procedure or more training being introduced. This has not proved to be effective in preventing the same mistakes being made again.

Pilot Error doesn't begin to explain why the errors were made or the conditions that allowed them to occur. A great deal of research has taken place into the underlying causes of aviation accidents and how we might prevent them in the future.

Why were those mistakes made? Recent research has identified that human error is inevitable. The question is; what can be done to minimise the consequences of those failings?

Aviation has recognised the need to introduce Threat and Error Management Training similar to Safety in Mind. The training is aimed at helping people develop strategies at the level of the individual, the team, the task, the workplace and the organisation. The objective being to prevent errors becoming significant, with far reaching consequences.

For other industries to make use of the lessons and research from aviation the ideas have to be adapted for the safety or risk managment needs of the particular industry and organisation.
